Comprehensive Care

Since August 2023, The Branch has provided transformative comprehensive care services for hundreds of people in Southeast Nashville by understanding the root causes of poverty and food insecurity, and helping our clients identify goals and connect to resources to get on a pathway to thriving.

Partnership on the Path to Thriving

Making life changes feels impossible for many people living in poverty. But our neighbors aren’t alone. We build trusting relationships that enable them to achieve holistic well-being. That spirit of partnership gets powerful results.

Compassion in Action

The Branch of Nashville provides comprehensive care through three support pathways.

1:1 Care Partner Coaches

Care partners provide one-on-one coaching to our neighbors. In a series of up to 12 meetings, participants work with their coach to:

  • Define goals
  • Create action steps
  • Identify obstacles
  • Come up with solutions

Resource Navigation

Resource navigation is similar to working with a coach, but it’s for a shorter period of time, focused on a single need. Through 1-2 collaborative meetings with our neighbors, we help them:

  • Assess need and determine eligibility for resources
  • Understand any application process for the resource/service [such as social security, SNAP, or legal representation]
  • Make an action plan
  • Communicate updates and answer questions

Love Your Neighbor Teams

A Love Your Neighbor Team is a small group of 4 to 6 volunteers who welcome immigrant families new to Nashville over a six-month period.

  • After receiving cultural literacy training, a team of 4-6 is introduced to the family.
  • They build nurturing, supportive relationships with one another, providing a sense of community to families adjusting to their new lives and surroundings.

Complex Problems Need Comprehensive Solutions

We call our approach “Comprehensive Care” because there’s no one-size-fits-all solution for systemic problems like poverty and food insecurity, especially for immigrants, single parents, asylees, older adults, and others in chronic poverty who come to The Branch for help.

“Why Don’t They Just Get a Job?”

The Branch (and our neighbors) know it’s not that simple.

  • They can’t get a job if they don’t have a work permit.
  • They can’t get a work permit if they don’t know the steps required to get a work permit, or if they don’t have legal representation.
  • They can’t get a job without stable housing, and they can’t afford housing without a job.
  • When they do find a job, they’ll need workforce readiness skills, reliable transportation and often childcare to meet the obligations of that job…

… And the list goes on.

The Process

Once a neighbor is interested in talking to a comprehensive team member more, this is the general process.

1.

Identify areas of need where changes are required in order to thrive, like housing, employment, immigration status, or education.

2.

Define action steps and goals, working with trusted “connectors” at The Branch to find resources and support necessary to make life changes.

3.

Take a trauma-informed approach to care and support that encourages community-building and healthy relationships.

4.

Adapt and adjust as things change. Celebrate when goals are achieved!
